STUDY A multicentre, randomised, open-label trial with a Bayesian adaptive enrichment design, evaluating EIT-guided PEEP titration versus a low PEEP/FiO2 table strategy in mechanically ventilated patients with moderate-to-severe ARDS. ClinicalTrials.gov record: https://clinicaltrials.gov/study/NCT06990477 RESEARCH QUESTION Do the clinical effects of EIT-guided PEEP differ by lung recruitability? Clinical benefit remains a research hypothesis, not an established result or a promise of benefit to an individual patient. DESIGN Up to 2,400 participants: a planned upper limit, not current enrolment. All enrolled patients receive an EIT assessment before randomisation. Randomisation is stratified by centre and alveolar collapse percentage, using a 25% threshold, with 1:1 allocation. Primary outcome: 28-day mortality. Secondary outcomes include 60-day mortality, ventilator-free days through day 28, and other ICU and hospital outcomes. Follow-up outcome collection extends to day 60. INCLUSION CRITERIA 1. Age 18–85 years. 2. Moderate-to-severe ARDS according to the Berlin definition: within 24 hours before screening, PaO2/FiO2 ≤ 200 mmHg at PEEP ≥ 5 cmH2O. 3. ARDS diagnosed within the preceding 72 hours. All exclusion criteria must also be assessed, and an appropriate informed-consent process completed by the research team.
